CVS survey Requirement

Survey Rules of CVS

To complete the CVS Pharmacy successfully, participants need to fulfill some requirements:

One person per receipt is allowed to take part in the survey.
The customer who wants to take part in the survey should be 18 years old or above.
The participant must be a legal resident of the United States including the District of Puerto Rico and Columbia.
One participant can only enter once in a month. Only one entry is allowed per family.
Employees of CVS Pharmacy and their family, households, relatives are not eligible to enter the survey.
A Purchase or payment is not necessary to enter the survey or sweepstakes. The purchases or payment will not increase the chance of winning the survey rewards.
There will be a random draw at the end of the month and winners will be announced after that.

Steps to complete CVS Survey

Go to the official survey site www.cvssurvey.com to enter the csv survey.
Select your preferred language.
Then enter 17-digits survey invitation code from your recent receipt.
You need to input the date of the visit and the time of the visit to the CVS store.
Also, rate the satisfaction factors asked there and answer the questions that are related to guest satisfaction issues asked there.
You will be on the reward page and you will be requested to enter the sweepstakes here.
Click YES and enter your personal details.
After that wait for the announcement of winners and you will be informed if you are the lucky draw sweepstakes winner.
